Do you suffer from any skin conditions such as; Eczema, Psoriasis, Impetigo, Acne, Skin Cancer, Vitiligo, Rosacea or Dermatitis?

Do you have any of the following medical conditions; Heart Disease, Heart Murmur, Pace Maker, High or Low Blood Pressure, HIV, Hepatitis, Haemophilia, Leukaemia, Asthma, Diabetes or Epilepsy?

Do you think you may be pregnant or are you currently breastfeeding?
